Our Support Coordination services
Support Coordinators collaborate with participants to find innovative and relevant ways to use their support funds to achieve their goals.
This will most likely entail assisting the individual with:
- A variety of mainstream, community, informal, and provider choices
- Choosing preferred options or providers
- Negotiation of the services and rates to be delivered, construct service agreements, and make service bookings with selected providers.
- As part of any quotable assistance, negotiate services and rates.
- Setting up any evaluations needed to establish the kind and amount of financing needed (e.g. an assessment to determine the type of complex home modifications required)
