Our Complex Home Modifications (CHM) services & our practitioners have years of experience successfully gaining approval and funding for well-planned and carefully considered home modifications under the National Disability Insurance Scheme (NDIS).

What do we do?

Your Complex Home Modification Assessor (CHMA) will do the following:

  • Meet with you and your family to learn more about your specific requirements.
  • Complete the initial house evaluation whenever it is convenient for you.
  • Submit the NDIS report (using the required CHM report template)
  • Go over the scope of work for the suggested changes and/or equipment script with the Project Manager.
  • Obtain any quotes from builders, contractors, or equipment suppliers that are not included in the planned Scope of Work.
  • Work closely with you to ensure that you understand the planned Scope of Works (drafted) and CHM Application before submitting them to the NDIS.
  • Submit your application to the NDIS as well as any other parties engaged in your care.
  • Respond to any further NDIS inquiries about the Scope of Work and suggestions.
  • Work closely with the Project Manager during the construction process.
  • Provide a final (follow-up) session to examine the adjustments and confirm that they were done to a high quality and that no difficulties or restrictions remain.
  • We will liaise with your selected Project Manager & NDIS to resolve any further needs or adjustments.
